Yes it has long since failed. So much was done wrong there, that one does not know where to start. The only strategy that ever had a chance was knocking over the regime as was done, but then immediately recruiting an Iraqi general to use the Iraqi army to provide security. As this happened, the USA could have drawn down its forces leaving an intact regime without Saddam. This is a useful discussion. I have certainly encountered individuals who have willfully chosen bad actions and continue to do so even when confronted.
